Liberty, NY – Sullivan County Public Health Services and the NYU School of Medicine have partnered to perform a health survey sponsored by the National Institutes of Health to better understand the health and dietary patterns of residents in Sullivan County and the prevalence of diabetes.
“Households will be randomly chosen to participate in this health survey,” said Public Health Director Nancy McGraw. “The survey takes only 5 to 10 minutes, and participants will receive a gift card by mail from NYU School of Medicine, plus the opportunity to participate in additional parts of the study for additional compensation.”
The survey is completely voluntary, and responses will be kept confidential. Only Sullivan County residents may participate and only one adult from your household may answer.
