Community Health – Partner Survey

The Sullivan County Department of Public Health is actively conducting our Community Health Assessment (CHA) to better understand the region’s health needs, priorities, and barriers. To ensure this process captures the real experiences of our residents, they are seeking input from community-based organizations and key stakeholders.

The community-based organizations are invited to complete a brief Community Partner Survey to share insights on the most pressing health issues affecting the populations you serve. This is part of a collaborative effort with Orange County and other Mid-Hudson Valley Departments of Health to gather comprehensive data across the region.

Why your input is important:

  • Your feedback will help identify priority areas for the next Community Health Improvement Plan (CHIP)
  • It ensures local voices and lived experiences are represented in regional health planning
  • Your participation supports a more equitable, informed, and community-driven approach to public health

Survey Details:

  • Takes approximately 5–10 minutes
  • Multiple responses from your organization are encouraged
  • Individual responses will remain confidential and reported only in summary
  • Deadline: June 16th, 2025

Multiple responses from the same organization are welcome and encouraged—we value the diverse perspectives each program within your organization can offer. Your input plays a vital role in shaping future public health strategies and improving health outcomes in our communities.

Please note: This survey is intended specifically for community-based organizations, not individual people. Feel free to share it with other local organizations within your network. In the coming weeks, we will also be distributing a Community Survey for all community members and a Provider Survey for healthcare professionals to ensure we gather input from every sector.
